Record of Divorce or Annulment
As required by MCL 333.2864, all divorces and annulments granted in Michigan must be reported to the State Registrar.
As part of the process for petitioning for a divorce, the petitioner or their attorney must properly complete a Record of Divorce or Annulment (MDHHS form DCH-0838) and submit it with the petition to the county clerk or other court official.
Completed records, signed by official, must be submitted to the State Registrar.
