Connected Communities Task Force

Connected Communities Task Force

The Michigan Department of Health and Human Services (MDHHS) established the Connected Communities Task Force (CCTF) to help guide the future of coordinated health and social care efforts across Michigan.

The task force brings together partners from across the state to identify opportunities to strengthen and sustain Community Information Exchange (CIE), Community Health Worker (CHW) services and related care coordination efforts.

Through collaboration and shared learning, the task force will help inform recommendations that support stronger community connections, improved access to services and long-term sustainability.

Task Force Priorities

The CCTF will focus on three key deliverables:

  •        Recommendations for integration of CIE, CHW and care coordination efforts.
  •        A return-on-investment framework.
  •        A financing options matrix to support sustainability of Michigan Hubs.

Task Force Members

The CCTF includes representatives with experience in health care, community-based services, care coordination and related fields. Task force members were selected through an open application as well as an appointment process.
