For Immediate Release: August 29, 2014

LANSING, Mich. – The Michigan Department of Community Health (MDCH) is requesting public comment on the amendments and home and community-based settings (HCBS) transition plans for the MI Choice Waiver as well as the Habilitation Supports Waiver.

The MI Choice Waiver amendment will allow for an increase in enrollment for the MI Choice Waiver program which provides home and community based services to eligible individuals that allows them to stay in their own home or another residential setting. The Habilitation Supports Waiver (HSW) amendment will allow individuals who are on the Healthy Michigan Plan to also receive HSW services and supports.

The purpose of the home and community-based settings transition plans is to assist MDCH in meeting federal requirements for providing home and community-based services in the most integrated settings, both residential and non-residential. Both amendments and the transition plans will be submitted to the federal Centers for Medicare and Medicaid Services for review and approval.
