LANSING, Mich. – The Michigan Department of Community Health (MDCH) recently announced a Request for Proposals (RFP) for the Children’s Behavioral Action Team (CBAT) Project. This RFP is for up to $3 million for a 19-month grant agreement with the possibility of ongoing funding to help youth with complex behavioral and physical needs. Funds for this project are part of the Mental Health and Wellness Commission’s recommendations.
MDCH is seeking a vendor who has successfully demonstrated the ability to create and implement a Children’s Behavioral Action Team (CBAT). The CBAT will transition youth out of institutions and into communities and provide training, consultation, and hands-on technical assistance to community providers. This project and will consist of the following full-time positions: a Clinical Project Coordinator (Licensed Master’s Level social worker), a Licensed Psychologist (full or limited license), a Board Certified Behavior Analyst, a Licensed Occupational Therapist, and two (2) Transition Managers (Licensed Master’s Level social workers). A third Transition Manager may need to be hired as the project progresses. Clinical Project Coordinator and Transition Managers must attend the MDCH sponsored three-day wraparound training within three (3) months of hire.
The CBAT will work in conjunction with a state-level CBAT Leadership Team, Hawthorn Center staff, multiple community providers such as Prepaid Inpatient Health Plans (PIHPs)/Community Mental Health Services Programs (CMHSPs), Michigan Department of Human Services, schools, courts, primary care and other physical health providers, and more. Additionally, families and guardians as well as the youth themselves will assist in creating unique, individualized community living arrangements and plans for treatment, supports and services to successfully maintain these youth in the community.
During the 19-month project, the CBAT will focus on a target population consisting of 25 youth ages five to 18 identified by the department to have had multiple hospitalizations and without a stable community living arrangement.
Applications must be submitted by January 9, 2015, at 3 p.m. EST to be eligible for consideration. The award will be announced by January 19, 2015. Only electronic applications will be accepted.
