Welcome to School Nurse Nexus. On this webpage school nurses can find information to support students with asthma and other chronic conditions. 

The School Nurse Nexus program links school nurses to resources offered by Medicaid health plans in Michigan. Its goal is to improve care coordination, enhance communication between schools and health plans, and increase access to care.

Michigan’s School Nurse Nexus was modeled after Missouri’s School Nurse Link program. We greatly appreciate the Missouri Asthma Prevention and Control Program’s guidance and assistance.

Why Connect

Care coordination! Each participating health plan provides information and resources that will support your students by helping you:  

  • Communicate with physicians and other health care providers
  • Obtain transportation for medical appointments or pharmacy visits 
  • Make appointments with specialists and primary care providers
  • Make referrals for self-management education, home visit programs or special programs

Preparing to Connect with a Health Plan

  • Which health plan to call? Parents/guardians should have a health insurance card that shows the name of the health plan.
  • Parent Permission. It is recommended that you get parent permission to contact the health plan so you can freely share information.
  • What’s needed. You will need the student’s full name and date of birth when you contact the health plan.

Parent/Guardian Permission

It is recommended that you get parent permission prior to contacting the health plan. Policies regarding documentation of permission may vary by school district, so make sure to follow your district’s policies.

The Missouri School Nurse Link program has developed a FERPA consent template you can use as guidance to develop your own consent form.

    • Hours: 8 a.m.–5 p.m., Monday–Friday. A member advocate can answer questions about your coverage, help find a doctor or help with an appointment

    Special Programs

    UnitedHealthcare Community Plan offers some special programs for which children and families may be eligible.

    Transportation

    • If you do not have a ride to get medical care and services, we provide transportation or gas reimbursement for you. Call MTM reservation line at 888777-4065 or visit: www.mtm-inc.net or download the MTM mobile app to request transportation.

    Dental

    • The Healthy Kids Dental program is offered statewide to those who get Medicaid and are ages 0 through 20 years. You will be enrolled automatically and will get an identification card from the dental plan.

    Mental Health

    Assurance Wireless Mobile Phone

    • As a UnitedHealthcare member or guardian of a UnitedHealthcare member, you may be eligible for mobile phone service through Assurance Wireless.

    UHC Doctor Chat

    • As a UnitedHealthcare Community Plan member, you can video chat with a real doctor from your phone, tablet or computer with UHC Doctor Chat.

    Pregnancy Care

    Prenatal care, postpartum care and dental services are covered when you are expecting. There are also the following programs that are available to help you have a healthy pregnancy:

    • Doula Services 
    • Healthy First Steps Program–earn free rewards 
    • Maternal Infant Health Program (MIHP)

    Priority Health, Medicaid

    Priority Health, Medicaid offers care management for various pediatric conditions including Asthma. Care management can work with a child’s family and care team to collaborate on the member’s care to ensure that the child’s Asthma is well managed.

    Care Management Collaboration for Asthma:

    The Priority Health School Nurse Liaison can collaborate with the school nurse team and members’ family to: 

    • Provider navigation, to find providers and specialists.
    • Guide the family through the healthcare system and answer questions.
    • Provide asthma education and care coordination. 

    Care Management

    Participating in care management can help a child’s family to better manage their health. If a family is interested in Priority Health, care management- they can reach out to our School Nurse Liaison.
