Eligibility

CCBHC services are available to any person in need, including but not limited to those with serious mental illness, serious emotional disturbances, long-term chronic addiction, mild or moderate mental illness, and substance use disorders.

• Any person with a behavioral health diagnosis is eligible for CCBHC services. All persons are entitled to walk into CCBHC clinics and be screened for service eligibility.

CCBHC clinics must service all individuals regardless of residency or ability to pay. If you reside out of state, CCBHC clinics are responsible for providing, at a minimum, crisis response, evaluation, and stabilization services and should have protocols developed for coordinating care across state lines. If you are uninsured or underinsured, CCBHCs have sliding fee scales and collect nominal fees based on income and family size. CCBHCs can also assist with insurance enrollment.

OnPoint

As a Michigan CCBHC Demonstration site, OnPoint will provide a robust set of coordinated, integrated, and comprehensive services to all persons with any mental illness or substance use disorder diagnosis, including 24/7/365 crisis responses services/mobile crisis services. OnPoint will transcend traditional access barriers within, between, and outside of our usual delivery systems, thus, participate in the positive development of Michigan’s public behavioral health system in Allegan County. OnPoint will align with and strengthen our existing integration and crisis service through the Michigan Crisis and Access Line (MiCAL) and partner with MDHHS in related initiatives that bolster funding, service optimization, and delivery of care for many more Michiganders.

OnPoint will continue to support accountability through financial and quality metric reporting, formal coordination with primary and other care settings to provide intensive case manager and transitions, linkage to social services, criminal justice/law enforcement, and educational systems; and an emphasis on providing services to veterans and active-duty service members.

Network180 (Kent County Community Mental Health Authority)

For Network180, being a CCBHC demonstration clinic will expand our ability to offer a coordinated, enhanced and more seamless experience to better provide behavioral health care and integrated primary care to community members seeking help. Being a demonstration site furthers our capacity to offer the right level of service at the right time, while continuing to make high-quality, evidence-based care our standard. We will now be able to serve anyone regardless of ability to pay, location, or insurance provider, and will have increased ability to prioritize our coordination of care efforts with community partners.

Community Mental Health of Ottawa County (CMHOC)

As a CCBHC, CMHOC will provide a comprehensive range of mental health and substance use services with an emphasis on increasing access to high quality care, integrating behavioral health with physical health care and promoting the use of evidence-based practices. CMHOC strives to enhance coordination of care and to continue to build strong relationships with community partners to better serve individuals receiving services.

West Michigan Community Mental Health (WMCMH)

WMCMH is a coordinator and provider of high-quality care for children and adults experiencing mental illness, intellectual and developmental disabilities, and substance use disorders. We serve Mason, Lake, and Oceana counties with a person-first approach. Our collaborative whole-person care improves individual recovery outcomes and long-term overall wellness.

As a Certified Behavioral Health Clinic, WMCMH provides intensive, community-based care, including 24-hour emergency crisis intervention services, targeted case management, and peer and family supports aligned with evidence-based practices for those who are underserved, have low incomes, have private insurance, are uninsured, are on Medicaid, and individuals who are active-duty military or veterans.

Barry County Community Mental Health Authority (BCCMHA)

Barry County Community Menal Health Authority, BCCMHA, provides treatment, recovery, and hope for individuals in the most effective, least restrictive manner to create the best possible environment for independence and wellness.  As a Certified Community Behavioral Health Center, CCBHC, we provide comprehensive behavioral health, substance use disorder services, and physical health care to all ages and provide care in a person-centered, family-driven, youth-guided manner. As a CCBHC, we can increase access to care, support people in crisis, and provide treatment through expansion of services to all individuals, regardless of insurance, residency, or ability to pay.

BCCMHA ensures that individuals, families, and youth have an empowered, decision-making role in how their care is delivered. We provide high quality, integrated services to meet the overall health needs of persons served, addressing both physical and behavioral health needs. BCCMHA partners with local physicians, schools, police departments, and community resources to help improve quality of life for all persons served.

Integrated Services of Kalamazoo (ISK)

As a CCBHC, ISK will consistently use evidence-based (proven or predictable) practices and increase access to high-quality care. We help people improve their health by making it easier to get into treatment, keep people from going into the hospital when they don’t need hospital care, make services available no matter your ability to pay or where you live, offer 24/7 mobile crisis response, integrate evidence-based addiction and mental health services, including medication-assisted treatment (MAT) for addiction, provide high-quality care, with timely access, quality reporting, and staffing and coordination of social services, criminal justice, and education systems. We also have built strong relationships between hospitals and community health care providers.

Pines Behavioral Health Services

At Pines Behavioral Health Services, we believe that each customer deserves to experience compassionate care that will lead to a more productive and healthier life. We are committed to providing behavioral health services to individuals and families of our community and will strive to assist them in reaching their highest quality of life. As a Certified Community Behavioral Health Clinic, the goal of Pines Behavioral Health Services is to meet the overall health needs of the clients served through improved access to a wide variety of services including 24/7 mobile crisis response and evidence-based addiction and mental health services. The integration of care approach is a key component to this model and all of our providers work together as a team on a client’s treatment plan and goals. Pivotal (Community Mental Health & Substance Abuse Services of St. Joseph County)

Pivotal’s mission is to enhance the lives of the citizens we serve by providing a range of individualized mental health, substance abuse, wellness, and recovery services. As a CCBHC Demonstration site, Pivotal now has the flexibility to serve all citizens with mental health or substance use needs within our catchment area and improve timely access to care. Pivotal maintains strong partnerships with local school systems, hospitals, law enforcement and numerous other community partners to provide strong integration across the care continuum.

Riverwood Center (Berrien County Community Mental Health Authority)

Berrien Mental Health Authority (BMHA) provides personalized effective behavioral health services to build a stronger and healthier community. By becoming a Certified Community Behavioral Health Clinic, BMHA’s goal has been to improve access to a wide variety of services to meet the overall health needs of consumers served. We strive to continuously improve the quality of care and treatment experience for all individuals through enhanced care coordination efforts, use of Evidenced Based (Best) Practices and partnerships with community resources and providers.

Summit Pointe

Summit Pointe is dedicated to providing the highest level of behavioral health care. This is achieved through consistent access to a wide range of behavioral health services in addition to comprehensive coordination of care with a wide assortment of community resources. Summit Pointe provides 24/7 access to services including screening, evaluation, crisis intervention, and psychiatric evaluation. Summit Pointe serves as a community hub for customers to receive connection to and engagement with social services including housing resources, transportation, food and nutrition, education, employment, and primary health services. Summit Pointe ensures access to behavioral healthcare needs including Substance Use, Mental Health, and Crisis Stabilization regardless of one’s ability-to-pay or their county of residence.

Certified Community Behavioral Health Clinics (CCBHCs)

Community Mental Health Authority of Clinton, Eaton, and Ingham Counties (CMHA-CEI)

As a Certified Community Behavioral Health Clinic (CCBHC), CMHA-CEI provides a comprehensive set of services used to increase access to care, support people in crisis, and provide treatment for those with behavioral health needs regardless of their insurance coverage. As a CCBHC, CMHA-CEI integrates additional services that focus on recovery, wellness, trauma-informed care, and physical-behavioral healthcare integration. 

Integration Efforts that have been sustained:

1) Partnership with local FQHC in which one of their clinics is situated at main CMHA-CEI location and CMHA-CEI RN are Manager is embedded in clinic and acts as a liaison between the two entities.

2) Laboratory and pharmacy located at main CMHA-CEI location.

3) Partnership with local Mental Health Courts.

4) CMHA-CEI hospital liaison staff who coordinates with psychiatric hospitals.

5) Navigation staff who partner with local school districts.

6) Provision of Crisis Intervention Training to local police departments.

7) Behavioral Health Consultants embedded in 2 local primary care offices, which primarily serve the Medicaid or uninsured population.

LifeWays

LifeWays is a community mental health services program that provides a comprehensive set of services used to increase access to care, support people in crisis, and provide treatment for those with the most serious and complex behavioral health needs. As a Certified Community Behavioral Health Clinic (CCBHC) we serve anyone, regardless of their residence or insurance coverage. We treat adults and children with emotional/behavioral challenges, intellectual/developmental disabilities, and substance use issues. Our services focus on recovery, wellness, trauma-informed care, and physical-behavioral health care integration.

The Right Door for Hope, Recovery, and Wellness

The Right Door for Hope, Recovery and Wellness provides a comprehensive set of services as a Certified Community Behavioral Health Clinic.  Our focus is to increase access to services for people in crisis and for individuals who have behavioral health needs regardless of insurance coverage.

Integration Efforts include:

1) DCO established with Samaritas to provide Substance Use Disorder services in our community. Samaritas is the sole comprehensive SUD provider in Ionia County.

2) Care Coordination relationships with all primary care physicians in Ionia County.

3) Ongoing partnership with all Ionia County courts, including mental health court.

4) Integration with public school systems and Ionia Intermediate School District.

5) Continued focus on integration with local DHHS, Health Department, ISD, Sparrow Hospital and courts - monthly meetings, and ad-hoc as needed.

Saginaw County Community Mental Health

Saginaw County Community Mental Health's mission statement is that as a public manager of supports and services for citizens with mental illness, developmental disabilities and chemical dependency and their families, SCCMHA actively strives to develop a system of care and a community that values and embraces the potential and contributions or all individuals with disabilities. The CCBHC Demonstration allows SCCMHA as an agency to broaden its scope of practice to provide these services to all individuals in need not only those with serious mental illness, serious emotional disturbances, long term chronic addiction, mild or moderate mental illness, and substance use disorders, thus developing a system that serves all individuals regardless of residency or ability to pay.

The Substance Abuse and Mental Health Administration having funded SCCMHA directly for Primary Behavioral Health Integration (PBHCI) from 2014-2018 and awarding funding to MDHHS for Promoting Integration of Primary and Behavioral Health Care (PIPBHC) from 2018-2023, where SCCMHA served as one of three demonstration sites,  provided over nine years of grant resources and technical assistance to support integration between local primary care providers and the behavioral health services of SCCMHA.  At the conclusion of the nine years of SAMHSA grant resources at the end of this fiscal year, we are confident that CCBHC-D resources in combination with our status as a new Behavioral Health Home will carry forward most of the work we have been providing in the Behavioral Health and Primary Care Integration space.

Monroe Community Mental Health Authority

By becoming a Certified Community Behavioral Health Clinic, Monroe Community Mental Health’s goal has been to improve access to a wide variety of services to meet the overall health needs of the clients served. We have partnered with the local police departments, schools, and FQHC’s to improve the quality of life for all.

Washtenaw County Community Mental Health

To promote hope, recovery, resilience, and advance health equity in Washtenaw County by providing, high quality, integrated behavioral health services to adults and youth with intellectual/developmental disabilities, mental health and/or substance use needs. Thanks to CCBHC and Millage funding, we are able to provide behavioral health services to individuals in need regardless of severity, insurance status and ability to pay. Washtenaw County Community Mental Health has permanent service locations in Ann Arbor and Ypsilanti, but also provides services at partner sites across the county, including Dexter, Chelsea, Whitmore Lake, etc.

Arab Community Center for Economic and Social Services (ACCESS)

Becoming a Certified Community Behavioral Health Clinic is an important element in Access’s goal to improve access to a wide range of services to meet the overall health needs of the clients we serve. The integrated care approach is a key component to this model and all of our providers work together as a team to develop and implement a client’s treatment plan and goals. Access continues to partner with local police departments, schools, hospitals, and FQHC’s to improve the quality of life for all.

CNS Healthcare – Wayne County

Mental health access and treatment is central to both the CCBHC model, and CNS Healthcare. CNS is committed to providing high quality, timely integrated care services in a trauma informed and evidence-based manner. Our providers use a team-based treatment approach to assist individuals in achieving their health goals. We are active in the communities we serve, reaching out to the general public, but also underserved populations such as veterans, LGBTQIA+ communities, Native Americans, and children. Our outreach efforts focus on improving access and reducing the stigma sometimes associated with asking for help. We have also strengthened and expanded partnerships with crisis providers, law enforcement, schools, and Federally Qualified Health Centers, and more. We are committed to our persons-served, improving outcomes, lives, and our communities at large.

Development Centers

Development Centers’ participation in the MDHHS CCBHC Demonstration Expansion will greatly expand our coordination of care and increase access for all individuals in need of integrated behavioral health and substance use services. These services will be available to individualsand families regardless of their ability to pay. The initial SAMHSA CCBHC Expansion grant award allowed Development Centers to further decrease ER utilization by 32%, expand veterans' services, begin specialized physical health groups (i.e. Diabetes), and grow primary care screening/monitoring services to two sites, one on the east and west side of Detroit. As a city with one of the largest Black populations in America, the Detroit community is disproportionately impacted by a long history of interpersonal, systemic, and institutional racism. The MDHHS CCBHC Demonstration designation will allow Development Centers to increase access to care and enhance coordination with hospitals, emergency departments, law enforcement, and other community partners to better help those in our community achieve self-defined recovery. 

Elmhurst Home, Inc. (Community Mental Health & Substance Abuse Services of Wayne County)

The mission of Elmhurst Home, Inc. (EHI) is to provide person-centered, stage-matched, evidence-based, and Recovery Orientated Systems of Care.” EHI specializes in residential treatment, outpatient, recovery housing, and recovery services for persons with a substance use disorder including persons with co-occurring disorders. Our primary goal is to assist each person served to achieve and maintain recovery and to improve every aspect of their quality of life. As a CCBHC Demonstration site, EHI now has the flexibility to serve all citizens with mental health or substance use needs within our catchment area and improve timely access to care. EHI maintains strong partnerships with hospitals, law enforcement and numerous other community partners to provide strong integration across the care continuum.

The Guidance Center

The Guidance Center’s participation in the MDHHS CCBHC Demonstration Pilot enables us to provide behavioral healthcare services to all people with mental health or substance use disorders, regardless of their insurance or ability to pay, or the severity of their diagnoses. We have added more than 30 employees in CCBHC programs, resulting in increased service capacity. We opened three new CCBHC locations for a total of seven CCBHC locations throughout the Downriver region of Wayne County and one location in Detroit. Since 2018, we have served more than 12,000 people through CCBHC Expansion and Demonstration funding, including more than 340 without insurance. Since the outset of the first SAMHSA CCBHC Expansion grant, TGC expanded its Evidence-Based Practices to include key services such as Medication Assisted Treatment (MAT) for people with Opioid Use Disorders, Zero Suicide, and Parenting Through Change (PTC).

Southwest Counseling Solutions (SWCS)

As a Certified Community Behavioral Health Clinic (CCBHC), Southwest Counseling Solutions (SWCS) provides evidence-based, comprehensive array of services designed to create access, stabilize people in crisis and provide necessary treatment for those with the most serious, complex mental illnesses and addictions. Through coordination with other healthcare and social service providers (including hospitals, primary care, foster care child welfare, education and criminal justice), SWCS focuses on whole health and comprehensive access to a full range of medical, behavioral and supportive services. Any individual in need who comes to the SWCS will be served, regardless of the ability to pay. We offer crisis services 24/7 every day of the year and provide immediate screening and risk assessment for mental health, addictions and basic primary care needs. Our CCBHC also provides care for veterans that’s customized to their particular needs.

Easterseals MORC

For over 150 years, Easterseals MORC has been leading the way to 100% equity, inclusion, and access for people with disabilities, their families, and communities by providing comprehensive behavioral health services. Through the CCBHC Demonstration, we’ve improved access to care by increasing access points, reducing wait times, and implementing targeted outreach efforts to improve service engagement. We’ve enhanced coordination of care through increased collaborations and partnerships such as, primary care providers, substance use providers, and mobile crisis/crisis stabilization services, resulting in better individual treatment outcomes and a collective positive impact on the health of our populations served. With advancements in evidence-based practices and the team-based care model, we’ve expanded services to our most vulnerable populations, such as, individuals with trauma, substance use, and/or complex health conditions regardless of insurance type, place of residence, or ability to pay.

GHS has been a part of the local community for over 60 years. GHS is the primary Genesee County public service provider for individuals with mental illness, intellectual/developmental disabilities, and substance use disorders. Our mission is to support recovery, prevention, health and wellness of the body, mind and the community. In fiscal year 2022, GHS served over 12,000 individuals. By becoming, a Certified Community Behavioral Health Clinic we are able to broaden our scope and serve all individuals in Genesee County who are experiencing mental health or substance use challenges, regardless of severity of need, insurance status, and ability to pay.

Lapeer County Community Mental Health

Lapeer County Community Mental Health's (LCCMH) goal in joining the Certified Community Behavioral Health Clinic (CCBHC) model is to support access to high-quality, evidence-based, and comprehensive behavioral health services. LCCMH has been part of the community for over 75 years and has shifted to integrating mental health, substance use disorder, and physical health care. A key part of this shift is to streamline the process for individuals to receive support for whole body care in one place.

Sanilac County Community Mental Health

Sanilac County Community Mental Health Authority (Sanilac CMH) mission statement is enhancing lives, promoting independence and embracing recovery. Becoming a Certified Community Behavioral Health Clinic Demonstration site will allow Sanilac CMH to be able to support and enhance the delivery of clinical and integrated services that improve the overall health of residents of Sanilac County. Services and supports are available for any individual with a serious mental illness, serious emotional disturbance, mild or moderate mental illness, and substance use disorder. In addition to traditional behavioral healthcare services (like counseling and psychiatric care), Sanilac CMH along with our partners, using trauma informed care and evidence-based practices, offer a wide array of services to meet the health care needs of those we serve.

St. Clair County Community Mental Health

St. Clair County Community Mental Health (SCCCMH) provides services and supports to any person in need, including but not limited to those with serious mental illness, serious emotional disturbance, long-term chronic addiction, mild or moderate mental illness, and substance use disorders. Our goal is to journey with you and your family to a place of hope, achievement, and recovery. Services focus on your individual needs and wishes. By becoming a Certified Community Behavioral Health Clinic, SCCCMH has expanded access to services, offering a wide variety of evidence-based practices to meet the overall health needs of the clients served.
