Eligibility

CCBHC services are available to any person in need, including but not limited to those with serious mental illness, serious emotional disturbances, long-term chronic addiction, mild or moderate mental illness, and substance use disorders.

  • Any person with a behavioral health diagnosis is eligible for CCBHC services. All persons are entitled to walk into CCBHC clinics and be screened for service eligibility.

CCBHC clinics must service all individuals regardless of residency or ability to pay. If you reside out of state, CCBHC clinics are responsible for providing, at a minimum, crisis response, evaluation, and stabilization services and should have protocols developed for coordinating care across state lines. If you are uninsured or underinsured, CCBHCs have sliding fee scales and collect nominal fees based on income and family size. CCBHCs can also assist with insurance enrollment.

West Michigan Community Mental Health (WMCMH)

WMCMH is a coordinator and provider of high-quality care for children and adults experiencing mental illness, intellectual and developmental disabilities, and substance use disorders. We serve Mason, Lake, and Oceana counties with a person-first approach. Our collaborative whole-person care improves individual recovery outcomes and long-term overall wellness.

As a Certified Behavioral Health Clinic, WMCMH provides intensive, community-based care, including 24-hour emergency crisis intervention services, targeted case management, and peer and family supports aligned with evidence-based practices for those who are underserved, have low incomes, have private insurance, are uninsured, are on Medicaid, and individuals who are active-duty military or veterans.

Pivotal (Community Mental Health & Substance Abuse Services of St. Joseph County)

Pivotal’s mission is to enhance the lives of the citizens we serve by providing a range of individualized mental health, substance abuse, wellness, and recovery services. Community Mental Health Authority of Clinton, Eaton, and Ingham Counties (CMHA-CEI)

As a Certified Community Behavioral Health Clinic (CCBHC), CMHA-CEI provides a comprehensive set of services used to increase access to care, support people in crisis, and provide treatment for those with behavioral health needs regardless of their insurance coverage. As a CCBHC, CMHA-CEI integrates additional services that focus on recovery, wellness, trauma-informed care, and physical-behavioral healthcare integration. 
Integration Efforts that have been sustained:

  1. Partnership with local FQHC in which one of their clinics is situated at main CMHA-CEI location and CMHA-CEI Rn Care Manager is embedded in clinic and acts as a liaison between the two entities.
  2. Laboratory and pharmacy located at main CMHA-CEI location.
  3. Partnership with local Mental Health Courts.
  4. CMHA-CEI hospital liaison staff who coordinates with psychiatric hospitals.
  5. Navigation staff who partner with local school districts.
  6. Provision of Crisis Intervention Training to local police departments.
  7. Behavioral Health Consultants embedded in 2 local primary care offices, which primarily serve the Medicaid or uninsured population.

The Right Door for Hope, Recovery, and Wellness

The Right Door for Hope, Recovery and Wellness provides a comprehensive set of services as a Certified Community Behavioral Health Clinic. Our focus is to increase access to services for people in crisis and for individuals who have behavioral health needs regardless of insurance coverage. Integration Efforts include:

  1. DCO established with Samaritas to provide Substance Use Disorder services in our community. Samaritas is the sole comprehensive SUD provider in Ionia County.
  2. Care Coordination relationships with all primary care physicians in Ionia County.
  3. Ongoing partnership with all Ionia County courts, including mental health court.
  4. Integration with public school systems and Ionia Intermediate School District.
  5. Continued focus on integration with local DHHS, Health Department, ISD, Sparrow Hospital and courts - monthly meetings, and ad-hoc as needed.

Saginaw County Community Mental Health

Saginaw County Community Mental Health's mission statement is that as a public manager of supports and services for citizens with mental illness, developmental disabilities and chemical dependency and their families, SCCMHA actively strives to develop a system of care and a community that values and embraces the potential and contributions or all individuals with disabilities. The CCBHC Demonstration allows SCCMHA as an agency to broaden its scope of practice to provide these services to all individuals in need not only those with serious mental illness, serious emotional disturbances, long term chronic addiction, mild or moderate mental illness, and substance use disorders, thus developing a system that serves all individuals regardless of residency or ability to pay.

The Substance Abuse and Mental Health Administration having funded SCCMHA directly for Primary Behavioral Health Integration (PBHCI) from 2014-2018 and awarding funding to MDHHS for Promoting Integration of Primary and Behavioral Health Care (PIPBHC) from 2018-2023, where SCCMHA served as one of three demonstration sites, provided over nine years of grant resources and technical assistance to support integration between local primary care providers and the behavioral health services of SCCMHA. At the conclusion of the nine years of SAMHSA grant resources at the end of this fiscal year, we are confident that CCBHC-D resources in combination with our status as a new Behavioral Health Home will carry forward most of the work we have been providing in the Behavioral Health and Primary Care Integration space.

Washtenaw County Community Mental Health

To promote hope, recovery, resilience, and advance health equity in Washtenaw County by providing, high quality, integrated behavioral health services to adults and youth with intellectual/developmental disabilities, mental health and/or substance use needs. The Guidance Center

The Guidance Center’s participation in the MDHHS CCBHC Demonstration Pilot allows us to provide behavioral healthcare services to all people with mental health or substance use disorders, regardless of their insurance or ability to pay, or the degree of severity in their diagnostic criteria. We have added more than 30 employees in CCBHC programs, resulting in increased service capacity. We opened two new CCBHC locations for a total of six CCBHC locations throughout the Downriver region of Wayne County and one location in Detroit. Since 2018, we have served more than 12,000 people through CCBHC Expansion and Demonstration funding, including more than 340 without insurance. Since the outset of the first SAMHSA CCBHC Expansion grant, TGC expanded its Evidence-Based Practices to include key services such as Medication Assisted Treatment (MAT) for people with Opioid Use Disorders, Zero Suicide, and Parenting Through Change (PTC).

The CCBHC Demonstration has increased collaborations across multiple sectors, including enhancing our coordination and partnerships, such as, primary health providers, substance use providers, and mobile crisis/crisis stabilization services. Since CCBHC we have increased access to services through more access points, reduced wait-times and targeted outreach efforts to improve engagement into services. Demonstration has enhanced coordination of care, improving individual's outcomes in treatment, including a collective impact on the health of our populations served. Expansion of services for individuals with complex needs with advancements to Evidence-based practices, targeting our most vulnerable populations, such as, individuals with trauma, substance use, and/or complex, co-morbid health conditions. We have broadened our population served, regardless of insurance type, place of residence, or a person's ability to pay.

CNS Healthcare

Mental health access and treatment is central to both the CCBHC model and CNS Healthcare. CNS is committed to providing high quality, timely integrated care services in a trauma informed and evidence-based manner. Our providers use a team-based treatment approach to assist individuals in achieving their health goals. Macomb County Community Mental Health
