Eligibility
CCBHC services are available to any person in need, including but not limited to those with serious mental illness, serious emotional disturbances, long-term chronic addiction, mild or moderate mental illness, and substance use disorders.
- Any person with a behavioral health diagnosis is eligible for CCBHC services. All persons are entitled to walk into CCBHC clinics and be screened for service eligibility.
CCBHC clinics must service all individuals regardless of residency or ability to pay. If you reside out of state, CCBHC clinics are responsible for providing, at a minimum, crisis response, evaluation, and stabilization services and should have protocols developed for coordinating care across state lines. If you are uninsured or underinsured, CCBHCs have sliding fee scales and collect nominal fees based on income and family size. CCBHCs can also assist with insurance enrollment.

As a Michigan CCBHC Demonstration site, OnPoint will provide a robust set of coordinated, integrated, and comprehensive services to all persons with any mental illness or substance use disorder diagnosis, including 24/7/365 crisis responses services/mobile crisis services. OnPoint will transcend traditional access barriers within, between, and outside of our usual delivery systems, thus, participate in the positive development of Michigan’s public behavioral health system in Allegan County. Saginaw County Community Mental Health
Saginaw County Community Mental Health's mission statement is that as a public manager of supports and services for citizens with mental illness, developmental disabilities and chemical dependency and their families, SCCMHA actively strives to develop a system of care and a community that values and embraces the potential and contributions or all individuals with disabilities. The CCBHC Demonstration allows SCCMHA as an agency to broaden its scope of practice to provide these services to all individuals in need not only those with serious mental illness, serious emotional disturbances, long term chronic addiction, mild or moderate mental illness, and substance use disorders, thus developing a system that serves all individuals regardless of residency or ability to pay.
The Substance Abuse and Mental Health Administration having funded SCCMHA directly for Primary Behavioral Health Integration (PBHCI) from 2014-2018 and awarding funding to MDHHS for Promoting Integration of Primary and Behavioral Health Care (PIPBHC) from 2018-2023, where SCCMHA served as one of three demonstration sites, provided over nine years of grant resources and technical assistance to support integration between local primary care providers and the behavioral health services of SCCMHA. At the conclusion of the nine years of SAMHSA grant resources at the end of this fiscal year, we are confident that CCBHC-D resources in combination with our status as a new Behavioral Health Home will carry forward most of the work we have been providing in the Behavioral Health and Primary Care Integration space.

Development Centers
Development Centers’ participation in the MDHHS CCBHC Demonstration Expansion will greatly expand our coordination of care and increase access for all individuals in need of integrated behavioral health and substance use services. These services will be available to individualsand families regardless of their ability to pay. The initial SAMHSA CCBHC Expansion grant award allowed Development Centers to further decrease ER utilization by 32%, expand veterans' services, begin specialized physical health groups (i.e. Diabetes), and grow primary care screening/monitoring services to two sites, one on the east and west side of Detroit. As a city with one of the largest Black populations in America, the Detroit community is disproportionately impacted by a long history of interpersonal, systemic, and institutional racism. The MDHHS CCBHC Demonstration designation will allow Development Centers to increase access to care and enhance coordination with hospitals, emergency departments, law enforcement, and other community partners to better help those in our community achieve self-defined recovery.

The Guidance Center
The Guidance Center’s participation in the MDHHS CCBHC Demonstration Pilot enables us to provide behavioral healthcare services to all people with mental health or substance use disorders, regardless of their insurance or ability to pay, or the severity of their diagnoses. We have added more than 30 employees in CCBHC programs, resulting in increased service capacity. We opened three new CCBHC locations for a total of seven CCBHC locations throughout the Downriver region of Wayne County and one location in Detroit. Since 2018, we have served more than 12,000 people through CCBHC Expansion and Demonstration funding, including more than 340 without insurance. Since the outset of the first SAMHSA CCBHC Expansion grant, TGC expanded its Evidence-Based Practices to include key services such as Medication Assisted Treatment (MAT) for people with Opioid Use Disorders, Zero Suicide, and Parenting Through Change (PTC).

Southwest Counseling Solutions (SWCS)
As a Certified Community Behavioral Health Clinic (CCBHC), Southwest Counseling Solutions (SWCS) provides evidence-based, comprehensive array of services designed to create access, stabilize people in crisis and provide necessary treatment for those with the most serious, complex mental illnesses and addictions. Through coordination with other healthcare and social service providers (including hospitals, primary care, foster care child welfare, education and criminal justice), SWCS focuses on whole health and comprehensive access to a full range of medical, behavioral and supportive services. Any individual in need who comes to the SWCS will be served, regardless of the ability to pay. We offer crisis services 24/7 every day of the year and provide immediate screening and risk assessment for mental health, addictions and basic primary care needs. Our CCBHC also provides care for veterans that’s customized to their particular needs.

For over 150 years, Easterseals MORC has been leading the way to 100% equity, inclusion, and access for people with disabilities, their families, and communities by providing comprehensive behavioral health services. Through the CCBHC Demonstration, we’ve improved access to care by increasing access points, reducing wait times, and implementing targeted outreach efforts to improve service engagement. We’ve enhanced coordination of care through increased collaborations and partnerships such as, primary care providers, substance use providers, and mobile crisis/crisis stabilization services, resulting in better individual treatment outcomes and a collective positive impact on the health of our populations served. With advancements in evidence-based practices and the team-based care model, we’ve expanded services to our most vulnerable populations, such as, individuals with trauma, substance use, and/or complex health conditions regardless of insurance type, place of residence, or ability to pay.

Mental health access and treatment is central to both the CCBHC model and CNS Healthcare. CNS is committed to providing high quality, timely integrated care services in a trauma informed and evidence-based manner. Our providers use a team-based treatment approach to assist individuals in achieving their health goals.
We are active in the communities we serve, reaching out to the general public, but also underserved populations such as veterans, LGBTQIA+ communities, Native Americans, and children. Our outreach efforts focus on improving access and reducing the stigma sometimes associated with asking for help. Macomb County Community Mental Health, guided by the values, strengths, and informed choices of the people we serve, provides quality services which promote recovery, community participation, self-sufficiency, and independence. Through our Certified Community Behavioral Health Clinic initiatives, we have improved access to those in our community who previously had limited access to behavioral health care. We have partnered with MyCare Health Centers, a Federally Qualified Health Center (FQHC), one of which is co-located in our MCCMH North facility, local police departments, other social service providers, and local schools to improve the overall health of our community.

Genesee Health System (GHS)
GHS has been a part of the local community for over 60 years. GHS is the primary Genesee County public service provider for individuals with mental illness, intellectual/developmental disabilities, and substance use disorders. Our mission is to support recovery, prevention, health and wellness of the body, mind and the community. In fiscal year 2022, GHS served over 12,000 individuals. By becoming, a Certified Community Behavioral Health Clinic we are able to broaden our scope and serve all individuals in Genesee County who are experiencing mental health or substance use challenges, regardless of severity of need, insurance status, and ability to pay.

Sanilac County Community Mental Health
Sanilac County Community Mental Health Authority (Sanilac CMH) mission statement is enhancing lives, promoting independence and embracing recovery. Becoming a Certified Community Behavioral Health Clinic Demonstration site will allow Sanilac CMH to be able to support and enhance the delivery of clinical and integrated services that improve the overall health of residents of Sanilac County.
