Effective immediately, please use the attached Payment Agreement Amendment form. This is the original form that was used that consisted of 2 calculation lines. You can also access the form from the CSHCS database.
As a reminder: Please only use the current forms for the IRPA, IRPA Amendment and Application. Any obsolete forms will be returned.
