Cohort Two
The second cohort of SDOH Hub pilot sites launched in April 2024:
5: Lead Organization: United Way of Bay County
Jurisdiction: Bay County
Key Partners:
Overall goal: Efficiently connect individuals & families with all health, human, and social service resources through coordination with a navigator utilizing a technology hub to better meet SDOH needs.
Activities:
- Launch SDOH/CIE technology hub.
- Expand organizations in every sector in CIE technology platform.
- Increase capacity.
6: Lead Organization: Muskegon County Health Department
Jurisdiction: Muskegon County
Key Partners:
Overall goal: Address the persistent health disparities and achieve health equity in Muskegon County.
Activities:
- Connect and partner with current county and state community information exchange (CIE) efforts.
- Support capacity building efforts of Muskegon County community-based organizations.
- Pilot a community SDOH Hub in Muskegon County.
- Participate in Statewide CIE, health information exchange (HIE), and SDOH Hub efforts.
7: Lead Organization: Heart of West Michigan United Way
Jurisdiction: Kent County
Key Partners: Kent County Health Department, Health Net of West Michigan, Kent County Essential Needs Task Force
Overall Goal: Formalize key partnerships for service delivery and implement a Food Delivery Service pilot.
Activities:
- Formalize partnerships among key Kent County agencies and establish partnerships with essential community-based organizations.
- Engage Community Health Navigators (CHN) to identify populations of need and type of need.
- Engage food-providing partners and agencies.
- Provide training to CHNs on making program referrals.
- Establish a start to finish order and delivery process.
