CHRONICLE
Background
The Chronic Disease Registry Linking Electronic Health Record Data (CHRONICLE) is an automated, near-real-time surveillance system designed to capture clinical chronic disease information from electronic health records (EHRs) and support longitudinal chronic disease monitoring statewide.
CHRONICLE's timely and detailed data will improve our understanding of Michigan’s chronic disease burden, support the identification of priority populations, and drive the development of effective interventions to improve health outcomes and reduce health disparities.
With support from clinical partners, development of CHRONICLE began in 2021 and since has expanded to accommodate additional chronic conditions, data sources, user types, and functionalities. Currently, CHRONICLE captures information regarding select chronic conditions, including asthma, cardiovascular disease, chronic kidney disease, diabetes, high blood pressure, high cholesterol, sickle cell disease, and stroke.
CHRONICLE relies on voluntary disclosure by participating hospitals for sharing information from health care encounters where a diagnosis of a chronic condition or select acute events, such as a stroke, have been observed. CHRONICLE leverages Michigan's health information exchange (HIE) infrastructure and HL7 electronic health messages in the form of Admission, Discharge, Transfer (ADT) messages to collect patient- level health care data. As of November 2024, CHRONICLE began collecting chronic disease related information from emergency medical services data and death record data statewide. The combination of disparate data sources enables a more comprehensive chronic disease monitoring ecosystem for informing evidence-based public heath practice.
Evaluation of the system and data quality is in progress and will be completed before more widespread use of the system and aggregated data sharing. Additionally, the system continues to be refined as we prepare for statewide chronic disease reporting for conditions approved by the Michigan Chronic Disease Registry Advisory Board.
