logo for CHRONICLE (Chronic Disease Registry Linking Electronic Health Record Data)The Chronic Disease Registry Linking Electronic Health Record Data (CHRONICLE) is an automated, near-real-time surveillance system designed to link information from electronic health records on diagnosed hypertension and stroke events with other data sources within MDHHS and other State of Michigan agencies to support longitudinal chronic disease surveillance statewide.
 
With support from our partners and healthcare providers throughout Michigan, the establishment of the registry will improve our understanding of Michigan’s health and chronic conditions in near-real-time, support the identification of priority populations, and drive the development of effective interventions to improve health outcomes and reduce health disparities.
 
Planning for a new, electronic health record-based, chronic disease registry began in 2019 with funding support from the Centers for Disease Control and Prevention. Development of the CHRONICLE system began in 2021 and is ongoing as the system and collected data are evaluated for chronic disease surveillance use.

Currently, CHRONICLE is limited to stroke and hypertension data from partnering pilot sites in Southeast and Mid-Michigan. CHRONICLE leverages Michigan Health Information Network’s (MiHIN) existing infrastructure and statewide exchange of electronic health record messages in the form of Admission, Discharge, and Transfer (ADT) messages to collect data on healthcare encounters related to hypertension and stroke events.
 
As the system is further developed, additional data sources and chronic conditions will be included to expand CHRONICLE’s scope and application.

CHRONICLE Steering Committee

The CHRONICLE Steering Committee is a group comprised of critical stakeholders and community partners throughout Michigan with a passion for strengthening public health surveillance capacity for chronic diseases and improving health outcomes. The committee guides the strategic development of CHRONICLE and recommends activities to refine CHRONICLE and demonstrate public health use cases for chronic disease clinical data.
