BC3NP - Information for Health Care Providers
Become a BC3NP Healthcare Partner
The Breast and Cervical Cancer Control Navigation Program (BC3NP) is always looking for providers who will agree to provide care and become a primary care provider for women who come through the program. As a healthcare partner, you would deliver screening, diagnostic, and follow-up services to women that are eligible for the program.
To become a partner with the program, simply send an email with the following information to BC3NP@michigan.gov. A staff member from the state office will contact you with more information and next steps. Thanks for your interest!
- Clinic Name
- Clinic Address (including Street, City, Zip Code, and County)
- Clinic Fax Number
- Clinic Appointment Contact Person
- Clinic Telephone Number
