Become a BC3NP Healthcare Partner

The Breast and Cervical Cancer Control Navigation Program (BC3NP) is always looking for providers who will agree to provide care and become a primary care provider for women who come through the program. As a healthcare partner, you would deliver screening, diagnostic, and follow-up services to women that are eligible for the program.

To become a partner with the program, simply send an email with the following information to BC3NP@michigan.gov. A staff member from the state office will contact you with more information and next steps. Thanks for your interest!

  • Clinic Name
  • Clinic Address (including Street, City, Zip Code, and County)
  • Clinic Fax Number
  • Clinic Appointment Contact Person
  • Clinic Telephone Number

Make a Referral to the Program

If you have a patient you think is eligible for the Breast and Cervical Cancer Control Navigation Program (BC3NP), referring them is easy.

Simply have them call 1-844-446-8727 to see if they qualify. If they do, we will link them to the closest contracted location for program services.

To see a map of locations throughout Michigan where BC3NP services are available, visit BC3NP Locations (michigan.gov).

Thank you!
