If you are a PFOMS participant and need to update your name or mailing address we have in our records:

Please email us at MDHHS-PFOMS@Michigan.gov or call 844-464-7327, Monday through Friday, 9 a.m. to 6 p.m.

If you call and leave a voicemail, we will return your call. Our voicemail is monitored even when the project is not actively recruiting participants or having project office appointments.

When emailing us or leaving a voicemail please include:

  • Your previous name or address.
  • Year of birth.
  • Your new name or new address.
