MDHHS invites providers to the MI Health Link Provider Summit

LANSING, Mich. ­­­– The Michigan Department of Health and Human Services (MDHHS) is inviting Michigan healthcare providers to discuss the integrated care program, MI Health Link, and coordinating care for Medicare-Medicaid beneficiaries enrolled in the program.  

The MI Health Link Provider Summit will be held on Wednesday, Nov. 9, 2016, from 8 a.m. to 4:30 p.m. at the Kellogg Hotel & Conference Center located at 219 South Harrison Road, East Lansing, MI 48824. There is no cost to attend the MI Health Link Provider Summit.  

MI Health Link is a health care option for Michigan adults, ages 21 and older, who are enrolled in both Medicare and Medicaid, and live in the counties of Barry, Berrien, Branch, Calhoun, Cass, Kalamazoo, Macomb, St. Joseph, Van Buren, Wayne or any county in the Upper Peninsula. MI Health Link offers a broad range of medical and behavioral health services, pharmacy, home and community-based services and nursing home care, all in a single program designed to meet individual needs.

The Summit’s Keynote Speaker will be Mr. Tim Engelhardt from the Centers for Medicare and Medicaid Services (CMS), and features presentations and panel discussions on core concepts of the MI Health Link program such as person centered planning, care coordination, and service integration. The MI Health Link Provider Summit sessions will cover a range of topics associated with integrated care including nursing facility collaboration with health plans, coordinating physical and behavioral healthcare, engaging enrollees and strategies for enhancing transition planning for members transitioning between care settings. Attendees will also hear best practices for coordinating care from a MI Health Link Integrated Care Team.

This event is hosted by MDHHS in partnership with Health Care Association of Michigan, LeadingAge Michigan, Michigan Association of Community Mental Health Boards, Michigan Association of Health Plans, Michigan Assisted Living Association, Michigan Center for Assisted Living, Michigan County Medical Care Facilities Council, Michigan Health & Hospital Association, Michigan Primary Care Association, Michigan State Medical Society and Area Agencies on Aging Association of Michigan.
