Changes to Medicaid for Certain Non-citizens

New federal rules will change Medicaid eligibility for some non‑citizens. These changes begin on October 1, 2026.

Depending on your immigration status, you may keep full Medicaid coverage or move to Emergency Services Only (ESO) coverage.

Individuals whose coverage is affected will receive advance written notice before any change occurs.

 

Who will continue to receive full Medicaid coverage?

The following immigration statuses will continue to qualify for full Medicaid coverage, if all other eligibility requirements are met:

  •        Lawful permanent residents (LPRs) after the 5-year waiting period,
  •        Cuban/Haitian entrants,
  •        Compacts of Free Association migrants.

Most pregnant individuals and children under the age of 21 who lawfully reside in Michigan will also continue to receive full coverage.

Individuals in these categories do not need to take any action unless they receive a notice from the state.

 

Who will transition to Emergency Services Only (ESO) coverage?

Individuals whose immigration statuses do not fall within one of the categories above will transition to Emergency Services Only (ESO) coverage.

Emergency Services Only coverage includes treatment for emergency medical conditions. An emergency medical condition is one with severe symptoms that require immediate medical attention to prevent serious harm to health.

Emergency Services Only coverage does NOT cover:

  • Routine doctor visits
  • Preventive care
  • Ongoing treatment for chronic conditions
  • Most prescription medications
  • Non-emergency procedures

Individuals whose coverage will change to Emergency Services Only (ESO) will receive advance notice before the change takes effect.

A list of immigration statuses that may qualify for ESO coverage can be found below.

 

Who will no longer be eligible for any Medicaid coverage?

Anyone who no longer qualifies for full coverage Medicaid because of their immigration status will be switched to Emergency Services Only (ESO) coverage.  

 

What does this mean for others in my household?

Eligibility is determined separately for each person in your home. A change to your coverage does not automatically mean that coverage for your family member will also change.

 

How will I know if I’m affected?

If your coverage is changing, you will receive a written notice by mail.

The notice will explain:

  •        Changes to your coverage.
  •        Details about your new coverage.
  •        How to appeal the decision if the notice indicates you are no longer eligible for coverage.

 

How will MDHHS determine my eligibility?

MDHHS will use existing data sources to verify individuals’ immigration status whenever possible. If there is not enough information available to complete this verification, MDHHS will contact the individual and request additional documentation. It is important to respond to these requests promptly to avoid delays or interruptions in benefits.

If individuals have documents or information related to their immigration status, they should upload them to MI Bridges or work directly with their assigned case worker to provide any updates.

 

What do I need to do now?

  •        Affected members will get letters by mail, text, or email. Make sure to keep your contact information updated in MI Bridges so you don't miss important notices.
  •        Watch your mail and respond quickly to Medicaid renewal packets, letters from your health plan, or local county Medicaid office.
