MI-Mobile Health Coalition:	Michigan Mobile Health Corps:
The Coalition, led by MDHHS MI-TEaCH, is a network of Michigan-based mobile health units dedicated to advancing placed-based population health through infrastructure standards, data interoperability, policy change, cross functional training and partnership, community engagement and sustainability strategies.	The HealthCorps, led by Wayne State University, is a health care provider network advancing the ability to implement scalable place-based clinical health outcomes as a service delivery system.

Goals:

• To provide health and social services to Michigan communities that have been marginalized and are susceptible to poor health outcomes, who may not otherwise have access to these services.
• To address outcome gaps throughout the State’s most at risk locations, using the Social Vulnerability Index.
