Michigan Hub Sites

The Michigan Hubs pilot brought together communities, local organizations, public health partners, and regional collaboratives across the state to strengthen coordination between health care, public health, and social care systems. During the pilot period from January 2024 through September 2025, 19 organizations served as Hub leads or implementation partners, advancing community-driven strategies to improve service delivery and health outcomes in communities across Michigan.

As Michigan Hubs move into the next phase of development, the initiative is transitioning toward a more regional approach aligned with Michigan’s 10 Prosperity Regions. Each region will have a designated Regional Hub Lead responsible for coordinating partnerships, aligning strategies, and supporting regional priorities, while local organizations and community partners will continue to play an essential role in implementation and community engagement.

The information below highlights Michigan Hub sites and regional partnerships across the state, including lead organizations, service areas and priority focus areas. These efforts reflect the unique strengths, needs, and priorities of communities across Michigan and demonstrate the value of community-centered collaboration in advancing health and well-being.

Region 1: Upper Peninsula Michigan Hub

Counties represented: Alger, Baraga, Chippewa, Delta, Dickinson, Gogebic, Houghton, Iron, Keweenaw, Luce, Mackinac, Marquette, Menominee, Ontonagon, and Schoolcraft

Regional Lead Organization: Upper Peninsula Commission for Area Progress (UPCAP)

Local Lead Organizations: Chippewa County Health Department, Chippewa Luce Mackinac Community Action Agency (CLM CAA)
