The Health Access and Innovation Division leads collaborative initiatives that strengthen community partnerships, improve access to care and services, and advance innovative approaches to support the health and wellbeing of Michiganders. The Division works across sectors to support statewide and community initiatives and systems improvement efforts that address barriers to health.

MDHHS looks to the next phase of implementation for innovative Michigan Hubs

The Hubs Pilot (January 2024 through September 2025) has driven meaningful progress toward a coordinated, community-centered care system across Michigan. Michigan Hubs will continue to promote regional, multisector collaboration and provide the infrastructure for a sustainable framework that improves health and community wellbeing.

Michigan’s Roadmap to Healthy Communities

Together with partners across the state, a dedicated team developed, launched, and oversaw implementation of Michigan’s Roadmap to Healthy Communities, a statewide strategy to guide coordinated action across health care, community, and public-sector partners. Throughout implementation, the Department worked closely with community partners to advance shared priorities and respond to the rapidly evolving landscape. These collaborative efforts helped inform implementation activities, identify emerging opportunities and challenges, and support continued innovation in community-centered approaches across the state.

The next iteration of these efforts will build on the foundation established through the strategy, including ongoing implementation of Michigan Hubs and targeted interventions to improve outcomes across Michigan.
