The Michigan Department of Community Health (MDCH) has immediately suspended the license to practice as an audiologist of Lisa Ann Hamzik, Audiologist, based on her conviction in the Sixth Judicial Circuit Court, Oakland County, Michigan. Hamzik failed to report her conviction to MDCH as required by the Public Health Code.
On July 16, 2008, Hamzik, 43, was convicted of one felony count of embezzlement by agent greater than $1,000.00 but less than $20,000.00. As a result, Hamzik was sentenced to one day in jail, with credit for one day, placed on probation with terms for 11 months and ordered to pay $47,500.00 restitution.
On Nov. 26, 2008, MDCH issued an order summarily suspending Hamzik's license pursuant to the Public Health Code which provides for the mandatory summary suspension of a health professional upon the conviction of a felony, a misdemeanor punishable by imprisonment for a maximum term of two years, or a controlled substance-related conviction. An administrative hearing will be scheduled to address the status of Hamzik's license to practice as an audiologist.
