Interjurisdictional notifications are used to notify outside local health jurisdictions of patients on TB treatment who are transferring to new local health jurisdiction. The two forms should be used to assist in getting useful information to the right people in a timely manner. Updated forms can be found on the National TB Controllers Association website.
