July 15 2026 Reminder Community Transition Services are Available to MICH Enrollees
July 15, 2026: Nursing Facility Providers: This notification serves as a reminder that Community Transition Services (CTS) are available to eligible MI Coordinated Health (MICH) enrollees. MICH enrollees do not need to disenroll from MICH to access CTS. As stated in Section 1.3 of the MICH chapter of the Medicaid Provider Manual, the MICH Highly Integrated Dual Eligible Special Needs Plans (HIDE SNPs) are responsible for providing information to MICH enrollees regarding the availability of CTS, and for coordinating care and benefits as appropriate. The HIDE SNP is responsible for making referrals to CTS agencies when enrollees meet program criteria as defined in the Community Transition Services chapter of the Medicaid Provider Manual.
When a Nursing Facility (NF) resident is enrolled in MICH and is interested in accessing CTS, the NF must contact the enrollee’s HIDE SNP to request that the HIDE SNP care coordinator refer the enrollee to an appropriate CTS agency and begin coordinating services. The NF could also refer the enrollee directly to a CTS agency and the CTS agency will validate current program enrollment and contact the HIDE SNP for MICH enrollees to coordinate services, as part of their standard process.
