August 25 2025 Timely Admission and Discharge Reminder

August 25, 2025: Attention all Skilled Nursing Facility and Hospice Providers: This serves as a reminder of the importance of timely admission and discharge details in CHAMPS. This helps ensure the Medicaid beneficiary’s Program Enrollment Type (PET Code) information is accurate in CHAMPS.  For step-by-step instructions on how to enter an admission or discharge, review the provider tip Entering an Admission and Discharge. Providers are reminded to use the beneficiary’s Medicaid ID# when completing an admission to ensure that the demographic information correctly populates.

For step-by-step instructions on how to obtain the Medicaid ID# in CHAMPS, click on the following provider tip: Eligibility Inquiry. Search by combinations of First/Last Name, DOB and SSN.

  • If the beneficiary is found in CHAMPS, report the Medicaid ID# on the admission record. The beneficiary’s demographic details will auto-populate to the record, preventing clerical errors and the need for admission record corrections.
  •  If the beneficiary is not found in CHAMPS (no Medicaid ID# available), leave the Medicaid ID# field blank and ensure all details for the beneficiary are reported correctly on the admission record. Once Medicaid eligibility is determined, the Medicaid ID# will systematically attach to the record.

To help minimize the high volume of admission/discharge corrections from providers, please carefully review all record entries prior to submission. Ensure all fields are accurate and complete, including the details below:

  • Medicaid ID#
  • Name, Date of Birth, and Social Security Number (SSN)
  • Admission and Discharge Dates
  • National Provider Identifier (NPI)

Note: If there are demographic discrepancies between what is reported on the admission record versus the confirmed data in CHAMPS, the Medicaid ID# will not attach to the admission record. The provider will need to Contact Provider Support for a resolution.
