May 28 2026 Community Transition Services and Incarcerated Benefit Plan

May 28, 2026: Attention All Providers: If a beneficiary becomes incarcerated, they may be automatically disenrolled from the Community Transition Services (CTS) benefit plan (BP). Incarcerated beneficiaries are only eligible for inpatient hospital services.  Upon release from incarceration the beneficiary may automatically be enrolled into the Targeted Case Management Services for Recently Incarcerated Beneficiaries (TCM-INC) benefit plan. Providers are reminded to always verify the beneficiary’s eligibility prior to delivering services.

Beneficiaries have the freedom to choose between the CTS and TCM-INC benefit plan. To end the TCM-INC benefit plan providers should Contact MDHHS Provider Support.

To re-enroll the beneficiary back into the CTS benefit plan, providers must submit a request (e.g. Word document) to the secure file exchange that contains the Compass ID, a brief statement that the beneficiary is choosing to re-enroll in CTS, and the re-enrollment start date. Beneficiaries must meet eligibility requirements for the CTS benefit plan per policy outlined in the Medicaid Provider Manual in the Community Transition Services Chapter.
