Reentry Services for Incarcerated Individuals
Individuals leaving incarcerated settings are unlikely to have the resources needed to obtain health care and other services to support their well-being as they reintegrate into the community. The Michigan Department of Health and Human Services (MDHHS) offers programs and services specifically designed to support Medicaid-eligible individuals leaving incarceration.
These include Targeted Case Management (TCM) Services for Recently Incarcerated Beneficiaries, as well as Medicaid and/or CHIP services for youth leaving the juvenile justice system in accordance with the Consolidated Appropriations Act of 2023.
MDHHS also applied for and received approval for an 1115 Reentry Services Demonstration from the Centers for Medicare and Medicaid Services (CMS) that was unable to be implemented due to lack of funding appropriations in the Fiscal Year 2026 state budget.
Targeted Case Management (TCM) is designed to provide a comprehensive array of services that are appropriate to the needs of adult beneficiaries. These include comprehensive individual needs assessments and service plans, assistance accessing services, and coordination with medical providers and health plans. The Consolidated Appropriations Act of 2023 requires states to provide Medicaid and CHIP–related services for eligible juveniles before and after their release from a public institution. Michigan’s 1115 Reentry Services Demonstration Waiver would have allowed eligible incarcerated individuals to be enrolled in Medicaid up to 90 days before release from state prisons, local county jails, and juvenile facilities. The waiver included a targeted benefit package to support successful reentry into the community.
