MDHHS Summarily Suspends Adult Foster Care License in Kent County

MLHANSING, Mich. July 19, 2013 - The Michigan Department of Health and Human Services (MDHHS), Bureau of Children and Adult Licensing (BCAL) has issued an order of summary suspension and notice of intent to revoke the license of Kent County adult foster care small group home licensee Elizabeth Lewis, #AS410015631, 1701 Plainfield Ave., NE, Grand Rapids, Mich.

This action resulted from a recent investigation of a complaint of the adult foster care small group home.

The June 17 complaint investigation found violations of the Adult Foster Care Facility Licensing Act and administrative rules regarding the administrative capabilities of Elizabeth Lewis, resident protection, and staffing deficiencies. BCAL took emergency action to protect the health, welfare, and safety of vulnerable adults.

Effective 9:00 a.m. on July 19, the summary suspension order prohibits Elizabeth Lewis from operating an adult foster care large group home at 1701 Plainfield Ave., NE, Grand Rapids, Mich., or at any other address or location. Accordingly, it may not accept physically handicapped, developmentally disabled, mentally ill and aged adults for care after that date and time. The order also requires Elizabeth Lewis to inform all of the guardians of adults in her care that its license has been suspended and that it can no longer provide adult foster care.

Elizabeth Lewis has held a license to operate an adult foster care small group home since May 18, 1994. The most recent license was for six adults.

MDHHS is assisting in the effort to find safe and appropriate placement for the residents of this home.

Michigan law defines an adult foster care small group home as "an adult foster care facility with the approved capacity to receive 12 or fewer adults to be provided with foster care."

The licensee has the right to appeal this suspension. The case will be forwarded to the Michigan Administrative Hearing System (MAHS) for a hearing date.
