Process for International Notification of TB Cases
Migrant Clinicians Network (MCN) is a non-profit organization that provides bridge case management, support, technical assistance, and professional development to clinicians in health care delivery sites. MCN provides quality health care that increases access and reduces disparities for migrant farmworkers and other mobile underserved populations. One of MCNs programmatic focus is to provide continuity of care, or bridge case management, for mobile patients, via their Health Network.
- Migrant Clinicians Network (MCN) Enrollment Packet: If you are treating a patient that has clearance to travel and is expecting to leave the country, complete this form to enroll the patient in the MCN Health Network, to ensure their TB treatment is completed.
- TBNet Patient History and Brief Medical Information Form: Complete this form after enrolling your mobile patient in MCNs Health Network to notify international jurisdictions and MCN of upcoming travel.
Submit the following completed information to MCN and the MDHHS TB Control Program:

- TBNet Patient History and Brief Medical Information Form
- TB treatment plan
