Report a Concern

Have a concern related to health and safety during COVID-19 re-opening?

Health and safety is our number one priority during reopening. If you'd like to file a complaint, please review the information below to direct your concern to the appropriate authority or state agency. 

If you need help finding a complaint form, please contact the MDHHS COVID-19 Hotline at 1-888-535-6136. The hotline is also able to help fill out some complaint forms and connect you with a translator, if needed. The hotline is open Monday - Friday from 8 a.m. to 5 p.m.

  • If I am an employee concerned about the safety of my workplace, about my employer asking me to behave unsafely, or about my employer discriminating against me for refusing to work in a dangerous situation that could cause death or serious physical harm…

    I should call 855 SAFE C19 (855 723-3219) and/or file a complaint with MIOSHA.

  • If I am a member of the public with a complaint about businesses selling food (illness or not; though complaints about restaurants should be directed to local health departments as outlined below), or complaints about migrant labor housing, pesticides, animal and plant health, or gas stations…

    I should file a complaint with MDARD.

  • If I am concerned about potential violations by a Certified Nurse Aide (CNA) or at a Bureau of Community and Health Systems (BCHS) licensed facility (e.g. nursing homes, adult foster care homes, homes for the aged, camps, child care centers, etc.)…

    I should file a complaint with BCHS here or by email at BCHS-Complaints@michigan.gov

  • If I wish to file a complaint against a licensed occupational or health professional… 

    I should file a complaint with the Bureau of Professional Licensing (BPL) here or by email at BPL-Complaints@michigan.gov.  

  • If I wish to file a complaint against a licensed skilled trade professional or entity that may be in violation of the construction code, mechanical code, electrical code, etc…

    I should file a complaint with the Bureau of Construction Codes (BCC) with this form or by email at LARA-BCC-compliance@michigan.gov

  • If I am a member of the public and I believe a business or event should not be open, or people are not following social distancing guidelines in businesses or events (see “Workplace Safety for Employees” if you are an employee of the business in question)......

    I should contact my local law enforcement’s non-emergency line

  • If I am a member of the public with a food illness complaint about a restaurant, or have a complaint about a campground or public swimming pool (including access to restrooms, water fountains, or trash service)...

    I should contact the local health department where the restaurant, campground or pool is located.

  • If I am an employee and I have concerns about violations of my rights to paid sick leave or protected leave related to COVID-19, non-payment of wages or fringe benefits, or not being paid minimum wage or overtime... 

    I should file a complaint with the Wage and Hour Division

  • If I am a member of the public and want to report price-gouging, or scams involving my federal stimulus check... 

    I should file a complaint with the Attorney General’s Office
