WISEWOMAN Program

WISEWOMAN (Well-Integrated Screening and Evaluation for WOMen Across the Nation) helps participants understand their cardiovascular disease (CVD) risk factors and make healthy lifestyle choices.

WISEWOMAN is a federal program funded by the Centers for Disease Control and Prevention (CDC) and administered in Michigan by the Michigan Department of Health and Human Services.

  • The Michigan WISEWOMAN program has three main focus areas with relation to its participants.

    1. Identify and communicate risk factors for cardiovascular disease (CVD), stroke, diabetes, and other chronic diseases. Enrolled participants undergo a health assessment and risk reduction counseling. These services give participants the opportunity to decide if they would like to set a goal to improve their diet, physical activity, and other lifestyle behaviors.
       
    2. Encourage healthy lifestyle choices. Health coaches work with each participant to make lifestyle behavior changes that will help to prevent, delay, and control cardiovascular and other chronic diseases.
       
    3. Ensure access to health services to all communities.
    • Local WISEWOMAN agencies address access to health services by identifying women at higher risk of heart disease in their service areas and enrolling them in WISEWOMAN.
    • Once in the program, many participants find it difficult to think about making healthy lifestyle choices when they are having trouble paying rent, utilities, or buying food for their families. Michigan WISEWOMAN makes referrals to Michigan 211, where participants are connected to a community navigator who helps them access social services. Michigan WISEWOMAN also helps participants stay engaged in the program by offering tools to help minimize barriers.
       

    Who is eligible for the WISEWOMAN program?

    Those who are eligible for the WISEWOMAN program include individuals who are:

    Eligible clients who will benefit from this program include those who are at increased risk of heart disease and need preventive health screenings as part of their wellness routine.

  • Heart disease and cancer are the leading causes of death for Michigan women. To address this issue, the MDHHS Cancer Prevention and Control Section offers screening programs especially for women:

    The Cancer Section began the Women’s Health Partnership to partner with organizations to help promote these screening programs. Partner organizations:

    • enroll women for lifesaving health screenings;
    • engage women in services for themselves and their families; and
    • help women take advantage of free health services in the communities they serve.

  • Below are WISEWOMAN's program-approved procedures, rates, diagnosis codes, place of service, and revenue codes.

    WISEWOMAN is a federally funded program and not an insurance company. Our rates are based on the Centers for Medicare & Medicaid Services (CMS) at or below the Detroit rates, and our approved procedures are provided to us by our funder, the Centers for Disease Control and Prevention (CDC). Rates are reviewed bi-annually in January and July, but are subject to change at any time without notice.

    Reimbursement Rate Schedules and Billing Grids

    Please Note: Both the WISEWOMAN Program Unit Cost Reimbursement Rate Schedules and the WISEWOMAN Program Billing Grids utilize Current Procedural Terminology (CPT) codes, which are copyrighted by the American Medical Association and used by the Michigan Department of Health and Human Services under a licensing agreement.
