January 13 2026 Update Prior Authorization Fax Lines Restored

January 13, 2026: Attention All Providers: The following Michigan Medicaid Prior Authorization fax lines have been restored.
  • Main PA Fax (517) 335-0075
  • PDN and PA Changes Fax (517) 241-7813
  • PACER Fax (517) 763-0402

For a faster turn-around time, providers can always enter Prior Authorization using the Direct Data Entry (DDE) option in CHAMPS which is always the preferred method. Review the Prior Authorization Submission Video or refer to the additional Prior Authorization resources.
