Hepatitis C Outcome Reporting

The Michigan legislature approved funding for the Michigan Department of Health & Human Services (the Department) coverage of Hepatitis C direct acting anti-viral (DAA) medication treatment beginning March 1, 2016. Due to the high cost of these new medications, the legislature also mandated that the Department report treatment outcomes on an annual basis. Based on feedback from our provider community, the Department has developed an electronic template for prescribers to use to regularly track and then periodically submit outcome information to the Department.   

Below is a link to the Hepatitis C TRACKING SHEET created to simplify prescriber Hepatitis C DAA tracking and outcome reporting. Prescribers should review records for patients treated in the past calendar year and update the tracking sheet as described in the Hepatitis C TRACKING INSTRUCTIONS document linked below.

Complete this form at your convenience during the year and submit by email no later than March 1 of the next calendar year. For assistance with sending information securely through email, contact the MDHHS Pharmacy Management Division at 517-241-2060.
