Facility/Agency/Organization (FAO)

A Facility / Agency / Organization (FAO) provider is an entity that provides health care services. An FAO includes Hospitals, Nursing Facilities, Laboratories, etc., and have a Type 2 NPI number associated to them. Providers who need to associate to an FAO will need to wait until the FAO enrollment application has been approved in CHAMPS.

MDHHS utilizes provider email address information entered in the CHAMPS provider enrollment application to communicate with providers. Providers are responsible for maintaining accurate and valid email address information within their CHAMPS provider enrollment information. If the email address information is out of date or incorrect, enrolled providers will want to modify their enrollment information and submit it for approval.


Electronic Visit Verification (EVV)

As outlined in MMP 24-11 for dates of service on and after April 1, 2024, MDHHS will require the submission of EVV data for certain home health care services (HHCS). For Home Health Agency providers who complete the CHAMPS enrollment they should move on to the next step and complete the HHAeXchange provider onboarding form. This will allow HHAeXchange to create the EVV provider portal and work with the Agency to begin the EVV process. The onboarding form must be completed even if the Home Health Agency plans to continue to use their existing EVV vendor.

Certified Community Behavioral Health Clinics (CCBHC) Demonstration Program

CCBHCs are non-profit or local government agencies that provide comprehensive and coordinated behavioral health services to all Michiganders, regardless of their insurance, ability to pay, place of residence, or age. CCBHCs are federally required to provide nine comprehensive behavioral health services, including 24/7 mobile crisis response and medication assisted treatment for substance use disorders.

 

Potential CCBHC Expansion
Providers seeking to participate in the demonstration must agree to meet all reporting timelines determined during expansion and outlined in the CCBHC Handbook, follow the CCBHC Demonstration Policy, meet all certification and operational requirements, visit Potential Provider Information webpage to learn more. CCBHCs currently participating in the demonstration may also submit requests to add new clinics or service areas. Barring a federal extension of the demonstration or MDHHS policy changes, the demonstration is set to end in September 2027.
 
Certification
CCBHCs must complete the MDHHS certification process to become a CCBHC under the CMS CCBHC Demonstration. The certification will be documented and monitored through the Behavioral Health Developmental Disabilities Administration (BHDDA) Customer Relationship Management database. Potential CCBHCs must provide justification of meeting CCBHC criteria and upload supporting documentation verifying that standards have been met. Certifications are valid for three years.
