Michigan Chronic Disease Registry Advisory Board Members

Membership and Application Process

Representatives from various groups are being sought to serve on the CDR Advisory Board to provide diverse perspectives that ensure the development of a robust chronic disease registry capable of supporting evidence-based solutions to challenges in addressing chronic diseases throughout Michigan. The CDR Advisory Board will consist of at least 12 individuals, including:

  1. One individual representing the Michigan Health and Hospital Association.
  2. Two individuals representing the department’s Public Health Administration.
  3. One individual representing a chronic disease organization.
  4. One individual representing a local health department.
  5. One individual representing the Michigan State Medical Society.
  6. One individual representing emergency medical services.
  7. One individual representing the chronic disease academic community.
  8. One individual representing a minority-serving, community-based organization.
  9. One individual representing a tribal health agency.
  10. One individual representing a health system or hospital in this state.
  11. One individual representing the general public.

A chair and a vice chair will provide leadership for the council. Members will be requested to serve for 3 years. Administrative support will be provided through the MDHHS Lifecourse Epidemiology and Genomics Division.

Individuals who are interested in serving on the Michigan CDR Advisory Board are expected to demonstrate knowledge, expertise, and/or personal experience in chronic diseases.

Applications will be reviewed by an MDHHS committee and approved by the MDHHS director or their representative. We anticipate that board member selections will be made by March 2025 at the latest.
