Propose a Condition

Proposing a Chronic Disease for Reporting

To propose that a chronic condition be reportable to MDHHS for public health monitoring, the first step is completion of a proposal package and submission to the Michigan Chronic Disease Registry (MiCDR) Advisory Board for review. The Board will evaluate the public health need, appropriateness of the definition, feasibility, and readiness for collecting the proposed condition. If the proposed condition is deemed ready for reporting, the board will prepare a final recommendation for the MDHHS Director. Statewide reporting requirements will take effect only after the approval by the MDHHS Director.

Completed proposals should be submitted by email to MDHHS-Chronicle@michigan.gov. Submitters will receive confirmation of receipt.
