CSHCS ALERT #5-2018 - Information sheet and survey

Dear Colleagues,
The Children’s Special Health Care Services Division is working in partnership with the University of Michigan – Child Health Evaluation and Research Center (CHEAR) to conduct a satisfaction survey among its clients that have received the Insurance Premium Payment Benefit. Late last week, 160 surveys to benefit recipients were distributed via mail to beneficiaries who received the benefit between October 1, 2015 and September 30, 2016. A second copy of the survey will be distributed on June 2nd to all participants as a reminder of our need for them to complete the survey.
Attached you will find a copy of the survey and an information sheet that describes the survey and process. Survey results are confidential and all data will be de-identified prior to sharing with MDHHS. We are making you aware of this activity in case you receive phone calls from clients who may have questions about the survey and/or its legitimacy. We hope you will encourage them to participate.
