Dear Colleagues:
In order make the process for requesting access to the Children’s Special Health Care Services application more secure we have added the Children’s Special Health Care Services form to the Database Security Application (DSA).
Some of you may have (or will be) attended the webinar training provided by our vendor. Please reference the attached instructions to order login to MILogin to access the Database Security Application (DSA) and submit a DSA Request for CSHCS. If you require additional instructions to submit the CSHCS form itself, you can access supplementary training materials within the in the DSA by selecting Training > Training View > then select the Children’s Special Health Care Services form from the drop-down list.
This a process that will be completed on an annual basis. A year from when your request is approved you will receive a renewal email notification to submit a renewal form within in the DSA to retain your access.
When submitting your DSA Request you will need to select role(s) from the list below. If you aren’t sure if you need access to both systems, please check with your supervisor prior to submitting the request.
LHD Roles:
Please submit your request within 60 days or you will risk losing your access to the CSHCS and/or CHASS application.
