Information on Scheduling and Submitting Results of an Inspection

Chain of events for Inspections: 
MDHHS will notify the local health department via automated email when an application and payment have been received accepted.  It is then the responsibility of the applicant to contact their local health department to schedule an inspection.  Once the inspection is complete, the local health department should go to theonline interface to indicate whether the body art facility has passed or failed their inspection and their reccomendation for licensure.  Remember, you will need the 'HI' number included in the initial email from MDHHS in order to utilize this online interface.
